Perfect Balance Day Spa has a new donation Policy as of February 2014. Perfect Balance Day Spa will match any amount up to $50.00 to make up your donation.

In order to receive a donation from Perfect Balance Day Spa we need the following information:

- Name of Event

- Organization Name

- Contact Person

- Letter with tax ID #

- Must sign off in our donation book
